Final Report Guidelines
If you receive a J. Patrick Barnes Grant, we require a final report be submitted at the conclusion of your study. Following is an outline of the information we ask you to include:
I. Project Details
- Title of Your Study
- Grant number (contact us if you don't know this)
- Study start date
- End date
II. Investigators
- Please list each investigator, providing his/her role, organization, and division or department for which this person works
- Have there been any changes since the start of the project?
III. Study Overview: Briefly restate
- Objectives
- Design
- Hypothesis and Outcome Measures
IV. Conclusions of your work.
- What is the most important finding?
V. Recommendations
- Implications for practice: What will you do at your institution as a result of this study? What will be put into practice?
- Implications for education: Will your staff be educated on your findings?
- Implications for research: Will there be further study?
VI. Dissemination of Findings
- What will you do to disseminate your findings outside your own institution?
- Will you apply to The DAISY Foundation for a Lynne Doll Grant?
- Please confirm that you will send your report to the Sigma Theta Tau Virginia Henderson Library for Nursing Research?
VII. Financial Summary: Please include a spreadsheet of actual spending vs. your submitted budget
- Grant amount
- Total cost of the study
- Balance of grant to be remitted to The DAISY Foundation
