There was a patient on Vineyard View Suites who had a length of stay of 45 days. The patient had a very complicated medical situation and had been in our facility many times in the past. When the many disciplines of his medical team discussed treatment options and goals of care with this patient, the patient was mainly concerned about getting home to see his granddaughter's graduation and a family wedding. To get the patient home to participate in these family events, would take an experienced Case Manager who would be able to coordinate the disciplines of care that this patient would need. Luckily for the patient and his family, Deanna, was the Case Manager for this patient. Final transitions of care arrangements were coordinated by Deanna that included education for the family, IV antibiotics and supplies, home health services, and wound vac therapy. Because the patient was dependent on an IV drip, the coordination of services needed to be seamless in order for the patient to make it home safely. I heard a couple weeks ago that this patient passed away, but according to his family, the patient was able to attend the family events that he wanted to live for. Deanna not only is an outstanding role model as a Case Manager, but she clearly made a meaningful difference in the life of this patient and his family.
