Linda recovered J in PACU after an open cholecystectomy. After 90 minutes, J developed sudden tachycardia and hypotension. Assessment by the surgeon determined the problem to be fluid imbalance. Linda continued to monitor the patient and give fluid boluses. J showed no symptoms of overt bleeding and CBC results were reasonable, however, he showed little improvement after fluid challenges. Using critical thinking skills, Linda extended J's time in PACU to keep an eye on him despite the surgeons' wishes to transfer the patient to a regular Med/Surg Floor. Linda encouraged the surgeon to reassess J and reconsider his transfer to ICU where he could be more closely monitored. One hour after J was admitted to ICU, he was returned to surgery for internal bleeding.
