We recently had a 50 year old patient on IMC that requested hospice service, we were unable to get her to the Dove House, but had an appropriate bed for inpatient at Stella Maris. The patient lived with her daughter, due to limited income they do not have a car or funds for transportation. The daughter and mother were agreeable to Stella Maris, but the daughter would not be able to visit her daily due to transportation needs. They had no family, limited friends to assist and no church group. As the case manager, I called Caring Carroll and CATS, who could not assist because the patient would be out of Carroll County.

Melissa Frank was working and heard case management discussing the need with the PCC on IMC. Melissa called a church group she knew in another county who assists families in need. They offered to assist and pay for transportation daily to Stella Maris for the patient's daughter. Both the patient and daughter were moved by this kind gesture and had one less thing to worry about.
