Nancy Dereng, BSN, RN, GHS Care Coordination/Center for Family Medicine, is honored with this DAISY Award for Extraordinary Nurses. Nancy is recognized for her professionalism and patient advocacy.

Over an eight month period, Nancy assisted a patient in lowering her A1C from 14.2 to 6.8,a major triumph. The patient, however, had many obstacles to overcome before she could learn to manage her diabetes. Nancy not only provided education in the office, she enlisted the patient's family members and made several home visits. During these visits she also helped the patient understand how to make healthier food choices. When she saw that the patient's refrigerator was broken, she called the apartment manager and made sure the repair was made.
