Nicole Weaver is an excellent Nurse and should be recognized. Nicole goes above and beyond to provide excellent patient care on a daily basis. Nicole also continues to be a team player and has makes herself available to do a joint visit with staff and patients to meet our patient's needs. There are so many stories I could share about Nicole and one stands out recently.
One of our patients, who has been struggling for years with her chronic illnesses, needed to be transitioned to Hospice for long term care. Our patient had been through many nurses from other agencies and connected with Nicole when she began providing services. Nicole made herself available to be there for the joint meeting with Hospice and assisted in making the transition easy for our patient. Nicole's communication with the team was wonderful and it made a difference in the life of the patient.
