Paula is the Clinical Manager of the Case Management department. She and her team work diligently each day to manage admission stays, discharge planning and ensuring that patients have all of the supplies and equipment to meet their needs. Paula relentlessly works extra shifts to cover Case Management needs in the Emergency Room, meets with other external customers such as Population Health and Finance to collaborate and partner with initiatives and implement changes within her department to meet the goals of the organization.

Recently, Paula was the driving force in one special patient case. A patient at the end of life on an inpatient unit requested to go to her home country so that she would be surrounded by her family and friends when she passed away. Paula immediately arranged the transfer by working with the care team, transport and the admitting department to ensure this request was granted. Additionally, she able to get funding for the transport to alleviate any additional hardships to the family. Paula advocated relentlessly until the patient transferred home.
