Rebecca is always willing to assist, but one case this year stands out. She took a soldier under her wing and helped guide him through his new diagnosis of diabetes and has managed his care.
When the patient was diagnosed after an ICU admission for DKA, he was then presented to Ms. Conner at discharge for her assistance. Over the course of two months, Ms. Conner managed his care to reduce his A1C from 12.2 to 7.9. Ms. Conner worked with him diligently on education and understanding his new diagnosis. She laid out his plan of care and the next step with each visit to make him more comfortable in managing his diabetes and in navigating the medical world. Through her hard work and dedication to this soldier, he went from being labeled as non-compliant and unreceptive by no-showing his appointments and not following provider instructions, to being a fully informed patient, who embraced his diagnosis and has turned his health around. This patient has expressed to staff that he is extremely grateful for the care he has received from his medical team at Kamish.
Ms. Conner impacts the lives of every soldier she manages. Her coworkers, both nurse and providers, seek her out as a mentor. Not only is she a joy to have around, but her dedication to case managing a challenging population is unmatched and an impeccable model of nursing care.
