There is a patient on 5 South General with a LOS of greater than 200 days. This patient has endured surgeries, procedures, care plan changes, etc. Shelley recognized that while many of the regular staff know the patient's story, not everyone does. Shelley took the time and typed a time line of the 8 months of this patient's stay. The notes/time line is kept in the patient's chart box, so anyone involved in her care can access them. Members of the multidisciplinary team are very grateful of Shelley's efforts.
The patient's care is very complex and will require facility placement. Anyone covering for Social Service, Case Management, etc., now has a guide to follow in assisting with this patient's needs. This patient is on a vent and is unable to speak for herself and tell her story. Shelley is her voice and advocate.
Shelley committed herself to put this patient needs and goals first. She thought outside the box and developed a simple way to ensure no piece of the story was missed by any member of the team. Everyone involved in this patient's care is beyond grateful for Shelley's efforts.
