I would like to proudly recognize the extraordinary Medical Telemetry team for the exceptional diligence and collaboration demonstrated in the care of a patient who remained hospitalized for a total of 86 days.
Throughout this prolonged and complex hospitalization, the team remained consistently vigilant, proactive, and patient-centered. Despite the many risks associated with an extended length of stay, no preventable patient harm occurred, which is a direct reflection of this team’s commitment to safety, communication, and evidence-based practice.
Their daily attention to detail, strong interdisciplinary coordination, and unwavering focus on prevention created a safe healing environment for this vulnerable patient. Preventing harm over such an extended period of time requires not only clinical skill, but also persistence, teamwork, and genuine dedication – all of which this team demonstrated beautifully.
This group exemplifies the very spirit of the DAISY Award through their diligence, accountability, and compassionate care. They truly embody what it means to work together to keep patients safe.
