In 2024, the 7 Silver Ortho Spine & Neuro team faced a challenging year marked by increased patient harm events, including a rise in falls, a significant increase in Hospital-Acquired Pressure Injuries (HAPIs), higher rates of CAUTI, and declining patient experience scores. In response, the team demonstrated exceptional collaboration, clinical leadership, and a deep commitment to patient dignity by implementing targeted, evidence-based initiatives that produced measurable improvements in 2025.

The most impactful of these efforts was the Save Our Skin (S.O.S.) Initiative, developed to address the rise in HAPIs. Through comprehensive education, standardized prevention workflows, and strengthened interdisciplinary communication, the team achieved an 85% reduction in HAPI—despite a nearly 18% increase in hip fracture admissions, a population at inherently higher risk for skin breakdown. This initiative not only prevented significant patient harm but also improved healing trajectories and reduced the likelihood of long-term complications.

The team also launched the RN Initiated Fall Prevention Initiative, a proactive, nurse-driven model that empowered RNs to implement fall-prevention interventions immediately based on real-time assessment. This included early mobilization support, environmental safety modifications, individualized patient education, as well as staff education. As a result, the unit saw a substantial reduction in patient falls, with fewer injuries and improved patient confidence in their care team.

In addition to improving patient outcomes, the team strengthened staff safety through increased awareness and adherence to Safe Patient Handling practices. As a result, Safe Patient Handling injuries decreased dramatically—from 14 injuries in 2024 to just 3 in 2025. This reduction reflects the team’s heightened focus on safe mobility, proper equipment use, and a culture where protecting both patients and staff is a shared priority.

Importantly, the work developed by the 7 Silver Team has extended far beyond their own unit. Their tools, workflows, and education materials—originally created to address rising harm events—are now available to the entire organization through SharePoint. By formalizing their resources and making them accessible systemwide, the team created a sustainable model for improvement that other units can adopt. Their willingness to share best practices has accelerated learning across EvergreenHealth and positioned the organization to replicate their success in reducing harm and improving patient outcomes.

These initiatives contributed to broad improvements and measurable gains in Patient Experience, including higher scores in Nurse Communication, Responsiveness of Staff, Teamwork, Medication Communication, and Bedside Handoff.

The 7 Silver team’s ability to translate evidence into practice, advocate for vulnerable patients, and work cohesively across roles exemplifies the spirit of the DAISY Team Award. Their data-driven, compassionate approach has made a profound and lasting difference in patient outcomes, staff well-being, and the overall care environment at EvergreenHealth.
