Even the cardiologist was concerned. Adam's work completely reoriented this patient's care.
I would like to express my appreciation for the work of Adam Occaso, RN. While caring for a 78-year-old woman with lower abdominal pain, Adam obtained an EKG by protocol because she had also reported epigastric pain. The EKG was unremarkable. When the patient returned from CT scan, she again reported the epigastric pain, and Adam inquired whether I wanted another EKG. I ordered it without real suspicion for a cardiac etiology. The repeat EKG showed new precordial ST depressions, along with newly deepened flipped T's in 1 and AVL. Even the cardiologist was concerned. Adam's work completely reoriented this patient's care. The belatedly added troponins were in the 300's and 400's respectively. Adam then had to manage this patient's hypertension, increasing hypoxia, and new medication orders, including a nitroglycerine gtt. We transferred to RIH ED to meet the interventional cardiologist. Just before the patient's departure, the CT returned showing acute diverticulitis, and Adam got the antibiotics hung in time. It is a privilege to work with such a thoughtful, observant, and diligent professional as Adam Occaso, RN.
