This group embodies the very spirit of collaborative excellence. Their dedication to patient safety extends far beyond the traditional realm of nursing, creating a ripple effect that safeguards lives throughout our community. Their recent triumph in aligning the hospital's level 1 trauma center with a staggering 110 new re-verification standards stands as a beacon of their unwavering commitment. Each member contributes with unwavering focus: trauma registrars act as the data guardians, meticulously ensuring its accuracy - the foundation for effective performance improvement. Performance improvement coordinators continuously identify opportunities for improvement and translate those insights into tangible safety protocols. The injury prevention coordinator doesn't wait for tragedy to strike; they proactively reach out to our communities, educating them on how to prevent injuries in the first place. The Trauma nurse navigator has incorporated mental health screenings and interventions for trauma patients, a new holistic and evidenced-based practice to improve patient outcomes. Finally, the education coordinator ensures every healthcare provider involved in patient care, from nurses and doctors within the hospital to prehospital teams in the field, is armed with the latest best practices.
This team's collaborative spirit isn't just impressive; it's impactful. By functioning as a well-oiled machine, they directly influence patient outcomes, such as reducing VTEs, AKIs, surgical site infections, and overall mortality scores. These reductions of hospital events and mortality scores put Upstate in the top ten percent of trauma center performers in the country, according to TQIP benchmarking. From ensuring that accurate data informs decisions to actively preventing injuries before they occur, the Trauma Program Team exemplifies the true essence of the DAISY Team Award. This nomination is a recognition of their ongoing commitment to excellence.
