Alex got everyone involved who needed to be in order for him to make it home that night.
This past week, Alex was my preceptor and we were caring for a patient who had been in the unit for a long time. The patient had finally decided they wanted to go home on hospice ASAP and this occurred right around shift change. Alex immediately started coordinating everything. She got everyone involved who needed to be in order for him to make it home that night, she did not want him to have to wait until the next morning. Alex worked to ensure he had a better chance of being able to make it home before passing, which is what he really wanted. She stayed 2 hours past her shift to talk with the patient and his family, discussing and preparing them about what to expect with the hospice process, ambulance, and end of life. Because of her dedication, care, and compassion and how fast she organized things, the patient was able to make it home as he wished that evening before passing.
