This young patient had received a new cancer diagnosis. Amy had this patient multiple times during the patient’s stay. Prior to discharging the patient home, she noted that the patient and her spouse were not only overwhelmed by her diagnosis but also anxious about the care that needed to continue at home. She always makes a point to connect with her patients on a personal level through her care. She understands that being in the hospital can be scary, and providing that connection can establish trust and put the patient and their family at ease. In this case, that is exactly what happened. She ensured that the patient and her spouse had sufficient time to review the steps in wound care. Additionally, she wanted to ensure they had the necessary tools to organize the patient’s new and existing medications. She created a task list and med tracking sheet to help the patient and her spouse while at home. She would not consider this going above and beyond, but rather just doing what she does every day.
