On this super busy day, Amy Charbeneau had a full load, a patient coming from surgery, and a post op patient going home. The patient's husband was on his way and Amy went over the do's and don'ts after her surgery and tackled the med list. Amy went through sheets of information on medications the patient had taken prior, one page at a time and made sure the patient knew why she was taking them and the side effects she could expect. She then went through information on meds that were new to the patient, then in front of the patient she called the patients pharmacy and sent them the prescriptions, she even arranged the pick-up time for the patient. Amy had no computer between her and the patient, but she was sitting in the chair next to the patient helping to put the patient at ease. She then reassured the patient how well she would do at home but to call with any questions or concerns. She truly communicated authentically and at the level a patient could understand and formed a true relationship with the patient. That patient was well prepared for discharge to her home.
