Amy & "D" have been working together in the Green Health Center since April 2015. They work side by side day in and day out without any additional lab or clinical AOA support. They are extremely reliable employees who show daily commitment to their clinic and our organization. They continuously portray willingness to support other departments either in-person or remotely from the Greene Clinic (in-basket support, PCMH fail lists, etc.). Their dedication to their patients is outstanding, and they continue to go above and beyond to do whatever it takes to ensure their patients are taken care of- from squeezing in additional nurse and lab visits into their schedules and spending extra time facilitating resources for services at home, to follow up calls just to check in post-hospitalization and attending calling hours for patients that have passed away. Amy & "D" hold themselves accountable to providing the best possible care they can for their patients in the Greene Heath Center and treat every patient as family. These two nurses are the epitome of nursing excellence and the team-based approach to patient care and coordination.
