Amy Taylor had a patient who was discharged home before she was able to inform them of their outpatient dialysis schedule and antibiotic treatment during dialysis. Amy called the dialysis center to confirm they had the correct antibiotic on file. It turns out that the center did not have the correct medication available. Rather than call the patient and have them return to the hospital, Amy called a sister center in Bedford. The sister facility had the correct medication and was able to send it to the Bloomington center. Amy then called the patient to let them know what their dialysis schedule would be and what antibiotic treatment would be during his dialysis days. Amy went above and beyond for this patient in order to prevent readmission and ensure the patient made it to his next dialysis treatment on time.
