Anne's care coordination efforts are extraordinary. Anne is integral in relaying information about imaging studies and further work-up between the requesting and consulting physicians. After participating in a patient's care coordination, she brought information back to our nurses and presented about her condition to our staff. This was very high quality and showed a level of dedication to patient care and collegial exchange of information.
