During the recovery period, Annemarie identified a patient to be unsafe in returning to her home. Annemarie collaborated with the necessary resources in order to resolve the concerns so the patient would be safe after discharge. Annemarie kept the patient comfortable and safe during this time. She kept the patient updated and provided nourishment for her while the concerns were being resolved. Annemarie was instrumental in coordinating the patient's care.
