Aracelly ensured the coordination of the patient’s arrival home with home health oxygen service after the veteran was finally discharged from the ED to home.
Aracelly had a discharged patient from 4SW return with an acute change in condition and brought back to the ED. She helped coordinate with VA police to jump-start the patient's care as the lights were left on after having to be brought into the ED. She called the home health/O2 company to inform them of the delay as they met the Veteran at home with oxygen. The veteran was evaluated by the physician in the ED to make sure the patient was stable to be discharged home. Aracelly ensured the coordination of the patient’s arrival home with home health oxygen service after the veteran was finally discharged from the ED to home. She really helped the veteran feel less stressed and went “above and beyond”. She is amazing when caring for the dementia and mental health veterans who present to the ED.
During another case, she showed compassion for Jane Doe, whom she helped bathe, and afterward, the patient stated she felt wonderful. Another veteran she cared for was an immunocompromised transient and she cared for the patient with compassion and attention. She is always advocating for our vulnerable veterans and humanitarians.
