Arcare’s Chronic Care Management Team consists of RNs/LPNs who have ensured that patients are well-informed about their care conditions, medicines, and community resources. These nurses check on these patients frequently and have helped them get connected with food banks, utility bill assistance, and transportation as well as helped with affording their medications. For some of these patients, these nurses are all they have, and enjoy the calls and support this team provides. Several have assisted the homeless with housing, behavioral health issues, substance abuse, as well as domestic abuse ensuring they get help and support as well as ensuring their safety. These nurses have not only become a huge part of their patients' lives but also a liaison between patients and providers.
