Here at ARcare, our goal is to give our patients the best care possible. The services provided by the Coordinated Care Team help patients get the treatment they require to enhance and support better health. Our patients and communities benefit from a number of programs that our Chronic Disease Care Management nurses oversee. They pay careful consideration and work hard to guarantee that every patient gets the best care available. They have established a rapport with patients, who turn to them for guidance on how to treat a variety of chronic illnesses and to use Remote Physiological Monitoring to treat uncontrolled hypertension. In the past year, our department has seen numerous changes, including the inclusion of medical case management for the Ryan White clients in Arkansas. Rather than having a single nurse monitoring their HIV, clients/patients have one nurse helping to manage and support their care overall, whether they have five conditions or just one. These nurses are invaluable in helping patients and clients access community resources in addition to managing their chronic diseases. These may include food banks, housing help, and medication assistance programs. These nurses represent both the ideals of our organization and what nursing is all about. Their work is diligent and committed to the communities and patients they serve.
