We had an ICU patient who has bad lung and heart disease at age 89, and after maximal treatment she was supposed to taken off HFNC and placed on low flow cannula and allowed to pass away in peace if she went into distress. All orders for comfort were placed, and Ashley was asked to place patient on nasal cannula. Sometime later I came to check on the patient and she was still on HFNC. In my usual disgruntled state, I found Ashley and inquired as to why the orders had not been carried out. She said that the patient wished for a cupcake so she got her one before carrying out the orders. Why would she do such a thing that seemed so irrelevant in the big scheme of things? She did it because it was the most important thing for the patient at the moment. She probably may not realize it, but it is the greatness of her character that led her to act the way she did. When most of us would have made some excuse about non availability of cupcakes, Ashley made certain that the patient got her wish. In the moment of our conversation I (the patient's physician) felt very small as her great character towered over me. I feel humbled that nurses of this pedigree are working in our ICU.
