This RN case manager is a very dedicated and much-respected member of the VNA of Englewood Home Health team. The care she provides to her patients is exceptional and done so respectfully. This nurse is frequently requested by patients to manage their cases due to the skilled, compassionate, and patient-centered care that she provides. As her manager, I am very frequently in receipt of compliments and feedback related to the care provided by this nurse. This nurse tirelessly and consistently meets the needs of her patients and always exceeds their expectations. The care provided by this nurse is exceedingly compassionate, and she persistently goes above and beyond to promote good health, healing, and patient outcomes. One case in particular that comes to mind in regard to this nurse is that of a nearly 100-year-old elderly man. This gentleman, in particular, is not eligible for Medicaid or any county or state resources but does require assistance in order for his needs to be met. As this nurse had been case managing this patient for some time, it was easy for her to see that he was struggling. He struggled to get to the MD office and the grocery store to keep himself fed. This nurse assisted the patient in arranging care with an MD that makes house calls in order for his medical needs to be met. Once she identified that the patient did not have the ability to get to the grocery store, she instantly made it part of her weekly errands to go grocery shopping for this patient to ensure that he would always have meals to eat. She works collaboratively with the interdisciplinary team to provide care for this patient to ensure that all of his medical, psychological, and psychosocial needs are met – and met respectfully while allowing the patient to maintain his dignity and ability to be autonomous to the best of his ability. She is frequently called “an angel on earth” by her patients, and they feel so fortunate to have received care from this nurse. She epitomizes the VNA health group’s nursing philosophy and mission statement via the excellent patient-centered care she provides. This nurse is an unwavering advocate for her patients and truly treats her patients in a manner in which she would expect her family members to be treated. She is an invaluable asset to our team, organization, and patients. For these reasons and many more, I nominate Astrid Kretschmer, RN, for this DAISY Award.
