I am a daughter of one patient who comes every 2 months for continuing treatment of INR. Today Auayporn found out there were 2 records that could potentially cause an error when my mom needs nursing care, especially medication administration. She made sure that everything was solved with this double file. She made it all one file and contacted the relevant units for patient safety. In addition, she contacted the pharmacist to verify the documentation. She did all of this without any request from me or anyone but on the basis of patient safety.
