Augusto Tobi, RN

RN Augusto Tobi takes care to ensure that patients are safe in the home, and if the patient requires an alternate level of care, he follows up to ensure that the situation is escalated as needed. Augusto’s documentation is completed on time and he collaborates and coordinates timely with the health care team. Augusto also makes sure patients are visited according to operational needs, including volunteering to visit patients in multiple boroughs, including the Bronx and Queens, in addition to his assigned borough of Brooklyn. Augusto sees the whole clinical picture, and makes sure the health care team is aware as well.
