Barb was nominated by her leader for going above and beyond.
A hospital patient was being discharged home. The patient refused post-hospital rehab services because she needed to get home to take care of her 19-year-old special needs son. Barb coordinated care and support for the son so the patient could focus on her own health recovery journey.
Barb recognized that care extends beyond the individual patient and took action to address the social and caregiving challenges that were directly impacting the patient's health decisions.
