Bernie recently became involved in the care of a patient who has been in our hospital for an extended period of time: the patient was only recently placed on her service (vascular care). Bernie identified the patient had many needs, and she helped to facilitate an interdisciplinary meeting for this patient: leading the charge to help create an hourly plan of care together with the gentleman. Her direction, involvement, and leadership has moved the patient's care outcomes forward. She has built a relationship with the patient that has facilitated the entire care team in a positive direction. When the gentleman was transitioned out the new side of the hospital, Bernie committed to track his continued progress: even bringing him back to his former care unit daily for physical therapy. She brings him outside for fresh air on "good" days and arranges with other staff to help when she is unable to do so. Her consistency and follow-through with this patient are extraordinary, and she has gone above and beyond in displaying compassionate care. Bernie is certainly a role model for other nurses at St. Joseph's, and for that, we thank and recognize her.
