Bhumi had two correct bags to give to the patient, and hand-carried them to the patient’s home.
I was working remotely and called the mom of a patient, who was discharged. This mother was in tears. She stated she did not know anything about the new g-tube. After consoling and empathizing with mom, I called her home nurse who went to check on the patient. She texted me a picture of the new G-tube that was placed. Mom did not have proper connectors or correct bags at home. Trying to prevent a readmit, I contacted Bhumi who was the charge nurse. I explained the situation and she took the time (I know she was very busy) to not only answer all my calls but answer both mine and the home health nurse's questions. After texting Bhumi a picture of the attached G-tube, she was able to determine what was used during the patient’s hospitalization and what would work for the patient at home. Bhumi took pictures of the correct bags and connectors. Since the patient was discharged the day before, Bhumi had two correct bags to give to the patient, and hand-carried them to the patient’s home. She also assisted me with getting the correct reference and lot number. The coordinator ordered them and overnighted the delivery to the family’s home. Without Bhumi’s amazing assistance on what I’m sure was a very busy day, she was the reason we could keep the patient at home.
