As the night-covering social worker, I attempt to transition patients to skilled nursing centers in the evening. Brandi assisted in this goal with a 'can do' attitude 40 minutes before shift change. This patient had about five pages of medications and she went to work on the continued care orders like a pro! She called report in and left nothing for the incoming shift, except to wait for transport. She has a great, positive team attitude and is a great asset.
