Today is my discharge date and with their assistance, they made a difficult transition flow without obstacles which they helped me overcome. They were attentive to details and they very much helped speed up the process of finally going to my family for the recovery.
They explained the medications, the release paperwork, and when filling my prescriptions at a pharmacy near where I live, hit a snag. They called the pharmacy and assisted me in being able to provide medications even though my wallet and ID were missing.
Breann made the transition back to my family a simple one. And, she also assisted in the search for my lost wallet and phone. I only met her today and she left a lasting impression with her dedication to service above and beyond the call of duty. Therefore, she is the prime candidate for this DAISY Award.
