A very difficult patient, with complex medical and mental health issues, was admitted to the Medical Unit. Brenda was up to the challenge when this patient needed help the most and she found answers to every barrier to the discharge plan that was presented. Brenda made arrangements for meds, transportation and even found the patient a coat to wear when they left. Brenda got the patient a glucometer, oxygen and a bed even when she was not sure where the patient would go. All the patient had to do was call and one would be delivered. There were so many obstacles I am still amazed at all she accomplished.
The patient left on a Friday and was readmitted on a Monday. Brenda again stepped up to the plate with even more determination to develop a safe discharge plan for this extremely difficult patient. This patient was discussed at length by the Continuity of Care Team and instead of saying what would not work she stayed solution-focused at this uphill journey. More than 270 referrals were sent out that resulted in many return calls asking questions and setting in motion a potential placement.
I am nominating Brenda because of her dedication to finding a safe plan when this seemed impossible, for not allowing barriers stop progression of the plan and for always keeping the patient the center of focus.
