I am thrilled to nominate Brigida Wharton, nurse at Cherokee Urgent Care for the DAISY Award in recognition of her outstanding commitment to patient safety and exceptional critical thinking skills. 
 
Brigida demonstrated exceptional vigilance and clinical acumen while reviewing an order for ROCEPHIN IV. Despite the order indicating doses exceeding 1g, she noticed a discrepancy in the reconstitution instructions. The order specified a total volume of 38.88mL, while the reconstitution instructions for a 1g vial indicated a total volume of 25mL, raising concerns about the accuracy of the order.
 
In our Urgent Care setting, we do not have pharmacy oversight, and nurses are responsible for reconstituting medications. Brigida took the lead in ensuring the safe reconstitution of ROCEPHIN IV, engaging in a series of error prevention techniques, including STAR, ARCC, Stop & Resolve, and Team Member Checking. After collaborative discussion with her team, they determined the appropriate course of action. They decided to add 9.6mL of sterile water to each of the 2 vials, then further dilute the contents with 30mL of NS for injection for a total volume of 50mL for both vials, from which they could obtain the 38.88mL to administer.
 
Brigida not only identified a potential medication error but also took decisive action to ensure patient safety. Her quick thinking and attention to detail prevented a potentially harmful situation and demonstrated her unwavering dedication to the well-being of our patients.
 
Brigida collaborated with the pharmacy safety officer to work with the Epic Willow team to update the verbiage in the system to prevent similar errors in the future, showing her commitment to continuous improvement and patient safety.
 
Brigida's actions embody the spirit of the DAISY Award, and she is truly deserving of this recognition for her exceptional dedication to patient safety and clinical excellence.
