Her hands-on experience and suggestions were pivotal to the changes that were needed as we assessed the current needs and opportunities to improve their outcomes.
I had the opportunity to meet and work with the Readmissions Project Workgroup for the national collaboration from January through August. Her dedication and concern for the veterans from admission to discharge were evident in the way she advocated for change. Her hands-on experience and suggestions were pivotal to the changes that were needed as we assessed the current needs and opportunities to improve their outcomes.
She also volunteered to pilot the discharge rounding on her unit with social work and case management to Improve the discharge process of the veteran to streamline the patient and family needs as they transition home and make sure they have all the services or equipment they would need for a successful recovery at home.
She continues to participate when needed in staffing, not as a free charge, but carries full patient loads alongside her staff.
During a training session, we both attended uptown, I was amazed at the number of nurses that greeted her so warmly over the two days, informing me about being detailed to her unit, and her professionalism and genuine care for their well-being while working on her unit. Her detailed nurses were so appreciative of the care she gave to them over the period of time and as a nurse who has been detailed In the past, the experience they described Is not the norm.
The experience that I had with her recently, is the one that solidified my need for this nomination. I recognized an opportunity for Improvement with the LEAF utilization tool to turn inpatients. Right after the manager meeting, she had a quick huddle with her staff to educate them on our use of the LEAF turning tool, verify that all the 4th-floor patients that had a poor Braden scale were on the turning system and developed a plan of action/prevention WITH her staff for our at-risk patients.
Mrs. Harris embodies what nurse leadership Is supposed to be with her care and concern for both her staff and patients. She remains a staunch advocate for process Improvement. She provides a safe space for nurses that are detailed to her unit to practice, and she is an active participant in every Initiative In the development of better care.
It Is an honor to work with her and I hope you accept this nomination.
