Identifying factors that can be detrimental to patients' health within a clinic can be a challenge due to high patient turnover rates and the fast pace of this clinic. The patients that are seen in this clinic are at high risk of falling due to their comorbidities and the high-risk medications many patients take. The clinic is large, and patients often have to walk far to get to their room.  

Patients often get short of breath and tire out quickly, leading them to start feeling dizzy, putting them at risk of a fall. To help reduce falls in this clinic and to ensure patients' safety, the nurses collaborated with numerous departments to obtain funding for the installation of handrails in the clinic for patients to hold on to. The installation of handrails has not only reduced the number of falls in the clinic but also makes patients feel safer when walking in the clinic. When patients come to this clinic it can be scary because of the complex nature of the patient's illness. Having these new handrails helps ease the patients' worries when they are navigating the unit. Adding handrails shows how a small change makes a huge impact on the patient's comfort and safety.  

The nurses in this clinic are compassionate in the work they do and want to optimize the patients' health by collaborating with providers, pharmacist, social work, business staff, etc. They spend extra time with the patients educating on new medications, new disease processes, and procedures that they may be having. Through collaboration and education, patients have had positive outcomes in their health.
