I have known Carlos for 8 years now. He has been lead RN at Suite 350, Southeast COPC. Suite 350 clinic which
is a specialty clinic tucked in the South East Dallas community where we take care of people with diagnosis of HIV/AIDs and various stages of their illness. It caters to a panel of roughly 800 to 1000 patients. We have an even distribution of Caucasian,
Latino and African American populations. We see patients for all their primary care needs as well as complicated issues like mental illness, substance abuse and homelessness. Carlos has a great command of Spanish Language and is available to help at any moment for care of Spanish speaking only patients, saving valuable time and Parkland resources (translation services expenses). He is culturally competent to address needs of this diverse patient population originating from the surrounding ZIP codes that we barely lose any patient to Amelia Court or Bluitt Flowers clinics, once they come to Southeast Dallas. I believe RNs being first point of contact and both our RNs, Alma Moore, a recent DAISY Award Honoree, and Carlos Loreto have done a great job to retain the patients, which is essence of continuity of care and better outcomes. And less hospital/Emergency room
utilization and less re-admission rates.
Carlos is very compassionate and eager to help the patients, he is always ready to coordinate care effectively with case manager to have the patient needs taken car of in a timely manner. His initial evaluation and triage is very thorough and knowledgeable and saves the provider a lot of time to get to the point and take care of the patient.
He is always helpful when because of short staffing, almost without much prior notice his RN colleague is asked to report to another clinic and he manages the clinic by himself. He volunteers to provide Flu Vaccine to employees at Southeast Community Oriented Primary Care (COPC).
Best of all, our patients with multiple diagnoses of mental health problems/substance abuse/infections have a great trust in him and prefer to call the clinic for all their needs and advice rather than going to Emergency or hospital, which certainly reduces the utilization and cost at these places. He is always ready to wheel the patient down to X-ray or lab, call thepharmacy on patients behalf or talk to financial counselor or medication access specialist. He works diligently with two different providers at the clinic.
Please join me in acknowledging the contribution Carlos has made to this clinic as a Nurse Leader.
