Carmen was caring for a very challenging patient who had several health care needs and issues. Carmen worked very hard at ensuring this patient had all the supplies and medications he needed for discharge. When discharged, Carmen took the patient to our outpatient pharmacy and learned that they had not received the prescriptions for his medications. Carmen researched and found out the prescriptions were sent to an alternate outpatient pharmacy that would not honor hospital medication vouchers. Carmen wanted to ensure that this patient had their insulin available to treat his blood sugar at home. The patient expressed that he was unable to afford the medications. Carmen called the outpatient pharmacy and paid for the patient's medications. She also ensured his transport home stopped at the pharmacy to pick up his medications. She then placed a follow-up call to the patient to ensure he had received his medications and was settled in at home. Carmen displays a true dedication to her patients and ensures they are safe and more importantly cared for.
