I am writing to nominate the Saint John’s Case Management Team, led by Debbie Pacheco, RN, MSN, and Linda Panish, RN, CCM, for the DAISY Team Award. This extraordinary team truly embodies the mission of our ministry by serving all, especially the most vulnerable in our community.

In 2022, the case management department underwent a significant transition, reframing how their staff performed case management duties. The redesign to a triad model of care coordination elicited a range of responses, both positive and critical. Despite the initial uncertainty about maintaining quality service and managing patient length of stay, the team remained confident. They knew they had assembled some of the best case managers in Los Angeles. With ongoing education and support, they believed they could achieve their goals together.

Saint John’s not only met the deadlines for the extensive training of their social workers on new and expanded workflows but also became the only facility in the southern region ready by their go-live date. As the pilot facility working collaboratively with the newly formed Utilization Management Hub, they celebrated their successes together and learned from their challenges. Their caregivers, who had never worked in acute care within an inpatient setting, grew to become strong case managers through peer mentorship, daily training, and coaching.

By the end of 2022, their team had lowered our patient’s average length of stay variance for the Medicare population to -0.07, the lowest among all Providence ministries in California. This impressive achievement resulted from their hard work, adaptability to new workflow processes, continuous learning opportunities, and collaboration as a team, not only within their group but also with their physician and nurse partners. Their success benefits their ministry and, most importantly, provides safe and optimal outcomes for those we serve.

Physicians, clinical managers, and caregivers have all been impressed with the team's collaboration and accessibility in times of need. Their group and team efforts are highly visible and deserve recognition. I firmly believe that the Saint John’s Case Management Team truly deserves the DAISY Team Award for their exceptional commitment and dedication.
