While reviewing a discharge Medication Reconciliation with a patient new to OPIV, Cathy discovered the patient was not taking a medication on the list. Cathy also learned that the patient had not received prescriptions for other medications, including a new blood pressure medication that was on his list.
Cathy worked with Case Management and the Hospitalist Liaison to arrange for the missing prescriptions. Cathy took the time to review the complete list with the patient using teach-back and written instructions to confirm that the patient knew which medications to take. The patient thanked Cathy for helping, noting that it probably avoided another hospital visit.
Cathy is known for her diligent patient care and her focus on doing the right thing using Target Zero behaviors. She is a role model for compassion and patient advocacy and is often recognized by patients for her caring heart.
