Multiple attempts made via case managers to place this patient in a nursing home were unsuccessful. Chris was able to locate the sober house from where this patient was prior to his hospitalizations and STR. Chris called the place in CT and notified the case manager of his findings. Chris made the effort to reach his previous residence, so he now is going home.
