Cindy went above and beyond to clarify Lovenox orders for a patient who had already been discharged to a skilled nursing facility. The patient's wife had called to the unit concerned that he had not needed any Lovenox - just Coumadin. Cindy went back to his inpatient record to validate that the patient should also be on Lovenox. She reprinted the discharge orders and resent them to the facility. She also called to confirm that they received the information and to validate that he would receive the Lovenox as ordered. Cindy did this all on a weekend when limited resources were available. She was determined to insure his safety and prevent any complications in his recovery.
