Claudia continues to be an excellent patient advocate. She takes pride in her work and has an exceptional work ethic. Claudia has excellent relationships with the nine physicians of our practice and they rely on her to deliver excellent care to our patients. I would like to highlight a recent patient encounter where Claudia went above and beyond the call of duty for our patients. 

We had a patient that was recently hospitalized. Claudia was prepping his BP medication for renewal. When she called the patient to let him know the prescription was at the pharmacy ready to be picked up the patient stated he was unable to pick up the medication because he was unable to afford the $90 cost of the medication. He stated he was waiting for his check to come at the end of the month. Claudia knew if this patient did not take his medication, he would be back in the hospital. She called the pharmacy to pay. They would not take her credit card over the phone so she drove to Walgreens to pay for the prescription. Claudia then called the patient and told him the prescription was ready to be picked up she made arrangements. Claudia never told the patient she paid for the prescription because she did not want to embarrass him. I only recently found out about this by accident.

We recently had a patient having poor side effects (patient with frequent urination, interrupting daily activities) from a medication that was being disregarded and had been escalated multiple times. Being a strong patient advocate as she is, Claudia took the time out to listen to the family/patient hardships and tracked down the physician to engage in a conversation and establish a healthier medication protocol. The medication was adjusted and allowed the patient to resume his daily activities and feel more productive. The family was extremely grateful.

In another positive patient experience, Claudia was able to assist a patient who stated she passed out the night before and hit her head. Claudia advised the patient to go to ER and notified the attending. Claudia provided the patient with her personal cell phone. Claudia followed up and found the patient was at the hospital circling the ER but afraid to go in. Claudia was very patient and convinced her to seek treatment. The patient was found to be in Atrial Fibrillation and Claudia’s persistent follow-up enable the patient to have a positive outcome.
