Clyde Wyche, RN, has made a remarkable impact since taking over the Beaumont area, a region marked by high-acuity hospitalizations due to limited access to care and substantial social determinants of health barriers. From the beginning, Clyde has brought unwavering energy, compassion, and leadership. He quickly built trust with both Dialysis Kidney Center (DKC) teams and patients, serving as an advocate and support system for those most in need.
One exemplary story that demonstrates his dedication involves a former high-risk patient whom he continued checking on monthly. Though her risk level had decreased, Clyde remained engaged—something that proved to be critical. The patient reported feeling unsafe and infantilized at two separate care facilities. When she was placed in a rehab center for therapy, she was unexpectedly told she would be discharged.
Concerned, Clyde contacted the patient, who was unaware of any reason for discharge. Bedbound and still requiring occupational and physical therapy, the patient was at risk of being sent home with no means of care. Recognizing the potential for harm, Clyde contacted the facility’s social worker, who explained that the patient was ineligible to stay due to exhausted coverage and missing documentation. Acting quickly, Clyde reached out to the patient’s niece, her power of attorney, and worked with her to secure and submit the necessary documents.
This began on a Wednesday afternoon, and by Friday, thanks to Clyde’s persistence and coordination, the patient was accepted into a new facility. His actions prevented a hospital readmission and, most importantly, protected the patient from being discharged into an unsafe environment. This is just one example of how Clyde consistently goes above and beyond.
