It is my pleasure to nominate the nursing team at our clinic for their exceptional efforts and dedication in providing patient care during a critical medical emergency.

Mr. A,  a 70 year old, patient presented at our clinic with severe symptoms of chest discomfort, dizziness, and sweating. The patient's condition rapidly escalated, leading to a diagnosis of cardiac distress. Despite the immediate intervention by our medical team, the patient unfortunately experienced cardiac arrest.

Nursing Team's Response: Our nursing team, comprised of  Naveen, Annamma, Julee, Shynu, and Sheena, played a pivotal role during this emergency. They were actively involved in the initial vital sign assessments, the administration of emergency medications, and the initiation of a Code Blue response when the patient went into cardiac arrest at 3:20 PM.

Key Contributions:

1.    Quick Response: The nursing team responded swiftly to the evolving situation, ensuring that the necessary vital signs were recorded promptly and emergency medications were administered without delay.

Code Blue Activation: When the patient went into cardiac arrest, our nursing team efficiently initiated CPR and activated the Code Blue protocol, showcasing their preparedness and adherence to emergency procedures.

Collaboration: The nursing team worked seamlessly with the Code Blue team, which included doctors Dr.Yoosuf & Dr.Salim (Dr.Rola Bashir, Dr.Hansa, and Dr.Afthab Ali), fostering effective teamwork in the face of a challenging medical crisis.

Communication: Clear and effective communication was maintained with the administrative team, ensuring a smooth transfer process and coordination with Al Qassimi Hospital.

Recognition Recommendation: The nursing team's exceptional efforts in providing immediate and effective patient care during this critical incident demonstrate their commitment to the highest standards of nursing practice. Despite their maximum efforts to resuscitate the patient, they faced the unfortunate reality that the patient was beyond the stage of recovery.

In light of the above, I highly recommend recognizing Annamma, Julee, Shynu, and  Naveen for their outstanding contributions during this emergency. Their dedication, quick thinking, and teamwork embody the values of our clinic and reflect positively on the quality of patient care we provide.

Our nursing staff's exemplary performance during this challenging situation reflects their commitment to patient well-being and upholding the highest standards of healthcare. Recognizing their efforts would not only acknowledge their contributions but also motivate and inspire the entire healthcare team.

A 47-year-old female Patient came to the clinic with a complaint of CHEST PAIN and sweating. Immediately, the nursing team took the patient to the treatment room, did an initial assessment, and informed Dr. Yousuf, the on-duty doctor. The doctor advised Tropi and ECG.  ECG shows ST elevation v1-v3, Stat dose of Medicine PO given (aspirin 300mg stat, plavix 300mg stat, atorvastatin 80mg stat), and in IV cannula inserted Inj. Pantonix 40 mg. Contacted Alqassimi hospital and patient was closely monitored by the nursing team. Then the ambulance team came and rechecked the ECG and vitals; that time, the patient was relieved from pain and reduced blood pressure. The patient was transferred with the help of a national ambulance.

The following day, I contacted the patient, and the relative answered the call. The patient had undergone angioplasty. The family expressed their sincere gratitude for the dedicated teamwork of the doctors and the nursing team.
