They worked tirelessly to keep him comfortable, anticipating his needs and responding quickly to any signs of distress.
I am honored to nominate our CLC Team for the DAISY Award in recognition of the extraordinary compassion, teamwork, and clinical excellence they demonstrated while caring for a veteran with highly complex hospice needs.
When this gentleman was admitted to our unit, his care requirements exceeded what our team routinely manages. Rather than feeling intimidated, the team leaned in. They took the initiative to strengthen and refresh skills that were not part of their everyday practice, including tracheostomy care and advanced comfort-focused interventions. They asked questions, sought guidance, practiced together, and supported one another to ensure they could provide safe, competent, and confident care.
More importantly, they never lost sight of the person behind the diagnosis. They worked tirelessly to keep him comfortable, anticipating his needs and responding quickly to any signs of distress. They advocated for effective symptom management and maintained his dignity in every interaction. Their presence was calm, reassuring, and deeply compassionate.
The team extended that same care and empathy to his family. They created a space where loved ones felt welcomed, informed, and supported during one of the most difficult seasons of their lives. They explained the procedures, answered questions patiently, and ensured the family felt included and comforted. Their kindness made an immeasurable difference.
All of this was accomplished while continuing to provide high-quality care to more than 20 other veterans in the unit. Despite the added complexity and emotional weight of hospice care, the team remained committed to ensuring every veteran was safe, respected, and cared for with excellence. Their teamwork, communication, and dedication ensured that no one’s care was compromised.
This team exemplifies the spirit of the DAISY Award through clinical skill, compassionate presence, and unwavering dedication to our veterans and their families. It is with great respect and gratitude that I nominate them for this well-deserved recognition
