Crystal had a patient who came from an apartment with her son and was unable to return after being discharged from the hospital. The patient needed long-term care placement following the hospital stay due to a poor home environment, being unable to continue living with her son and being evicted. A nursing home placement was found, but due to bed bugs in the apartment where she was living, the patient did not have any clothing to bring with her to her new home placement. Crystal spoke with the patient’s granddaughter and provided her with resources that she can use to help support the patient moving forward. Crystal then brought in clothes from her own house and gave them to the patient so she had a couple of outfits to start with at the nursing home. Crystal went above and beyond to help the patient feel comfortable when she had to transition to a new living situation.
