Nomination from patient: Dawne did a great job and without her assistance I would have never felt better. She always went over and beyond what she was asked to do. She provided outstanding care to me. Supervisor input from this: Dawne also took this patient a hand held mirror to assist her becoming independent with her wound care to help with her self esteem.

Nomination from supervisor: Dawne cared for a very ill patient. Family was somewhat difficult for most staff members. Dawne was able to use her professionalism, skills and compassion to build rapport when patient was near end of life. Dawne was able to assess patient change inform physician, spouse and entire team on patient status. Family opted not to use hospice care. Dawne was willing to be with the patient and family during the final hour and for post mortem care. She role modeled appropriate, compassionate care to all involved. She served as a liaison to city personnel attending to a death at home and was a wonderful example of a professional, skilled, compassionate representative of FHC. She defiantly went above and beyond for this patient and family. She makes a difference!
