Rachel detected a patient’s deteriorating condition while she was performing her assessment for the day. She noticed his pulse was high, his blood pressure was low, and that he was overall feeling weak. Rachel notified me and the nurse in charge of the patient and we were able to have the doctor come to see the patient ASAP. The patient ended up having a GI bleed and was able to be treated quickly and efficiently thanks to Rachel. I am truly convinced Rachel saved that patient’s life. Not only that, but she saved the unit from having to call a rapid response or run a bigger code. She was comforting, efficient, and professional the entire time. Rachel is an outstanding student and has impressed me countless times!
