Deborah Stoeber exemplifies excellence in Nursing by her thorough assessment skills, prioritizing, patient centered interventions and evaluations. Deborah has been a tremendous advocate for her patients. After meeting case management guidelines and passing the Case Management exam, Deborah recently obtained her Certification in Case Management ahead of schedule, which will provide her the expertise and credibility as a Case Manager. There are many examples where Deborah not only provided outstanding care and support to the patient, but to the family as well. She case managed a young Active Duty Service Member diagnosed with cancer throughout the continuum of care, from several hospitalizations, outpatient services, homecare and Hospice. At each level of care, Deborah worked with the health care team, and was an active participant making sure that not only were options regarding medical care provided, but that emotional, psycho-social support systems, command support and financial supports were in place. There are many examples where Deborah went above and beyond. The patient required a monthly outpatient procedure, requiring ambulance transportation, which was originally an out of pocket expense for the family. She reviewed the medical necessity of the ambulance which allowed medical insurance coverage, saving the family hundreds of dollars a month. She took care of all ambulance arrangements regarding referrals, care coordination and scheduling, allowing the family more quality time.
She also, worked with Hospice in getting a pain patch for the patient, which was originally denied by Hospice. The pain patch provided the patient much improved pain relief, and comfort to the family. Deborah also worked closely with the Command, and Navy Wounded Warrior, making sure that legal paperwork was in place, honoring the patient's wishes and that patient/family were supported. This patient and his family would often contact Deborah, notifying her of treatment changes, calling for guidance, or just calling for support. This patient recently passed away at home peacefully with family at his bedside. It was a huge comfort to the patient and the family that all arrangements were already in place at the time of his death making a difficult situation less difficult and allowing them that support.
Deborah provided appropriate nursing interventions with compassion, and patient advocacy, which provided the patient/family with tremendous comfort and peace of mind, knowing that they had someone to help navigate the healthcare system with them and provide education and support.
Along with providing outstanding patient/family advocacy and support, Deborah also provides support to her peers by sharing information, precepting nurses, and by attending a Preceptor Education class.
Deborah exemplifies patient centered wholistic care, and provides consistent outstanding patient care and role modeling.
